Purchase Request Forms

Purchase Request Process

1. Check the Library Catalog:

Before filling out the form, check the library catalog to confirm whether the item you need is available.

2. Complete the Purchase Request Form:

Fill out all required fields on the form. Be sure to include specific details such as:

Title of the item

Author

Publisher/Source

Format – print book or database

ISBN (if applicable)

Edition or year of publication

Reason for the request (why it's needed for your research or course)

3. Forward the Request for Academic Approval:

Please forward the completed form to your department chair and/or dean for review/sign-off. 

4. Email the completed, approved request to the library at acquisitions@vsu.edu.

5. Course Reserve (if applicable):

If your request is for a course reserve (materials to be made available for your course), make sure to use the course reserve form instead of the purchase request form.  The course reserve form is available at the Information Services Desk.

6. Follow-Up:

After submission, follow up to ensure the request is processed and to track the status of approval and purchase.
